Provider Portal: Sign-in Help

Need Help Signing In?

​​For Providers: If you are a provider and need help signing in, click the "Providers Sign In" button and follow the instructions.

​​For GCPHN Staff: If you are staff for the GCPHN, use the "GCPHN Staff Sign In" button to log in.

First Time Logging In

​​Activation Process: If this is your first time using the portal, you will need to finalize your account configuration. Please email providerportal@gcphn.com.au or give us a call at (07) 5635 2455 if you require any assistance.

Performance Reports

This page displays a list of your organization's Performance Reports and their current status.

1. To Find a Specific Performance Report: Use the search bar if you know the name or a keyword.

2. To Sort the List: Click on the arrows next to a column header, like "Due Date," to organize the list to your preference.

3. To Take Action on a Performance Report: Click the name of the report. This will lead you to a new page where you can provide the required information or documentation. Look for items with the status "Ready for Provider Input" to see what you need to work on.

Filling out the Performance Report

​The Performance Report is comprised of 4 tabs:

    1 Contract Details: Shows information about the Provider and gives a summary of the sub-reports. 

    2 Performance Data: This tab contains all of the Data Indicators (with metric values). 

    3. Discussion Points: This tab contains all the qualitative information needed in the report. 

    4. Review: Once you are done with your entries, you can review and submit your report.

Take a Tip:  As you work on your report, you can save your progress at any time. Just follow the on-screen prompts and bottom navigation buttons to guide you through each step.

Contract Deliverables

This guide will help you navigate and understand the Contract Deliverables page on the Provider Portal. This page displays a list of your organization's deliverables and their current status.

1. To Find a Specific Deliverable: Use the search bar if you know the name or a keyword.

3. To Take Action on a Deliverable: Click the name of the deliverable. This will lead you to a new page where you can provide the required information or documentation. Look for items with the status "Ready for Provider Input" to see what you need to work on.

User Registration

This guide will walk you through the steps to register a new user on the Provider Portal. This form is used to nominate someone within your organization to gain access to the system.

UserRegistration


1. Access the Form: Navigate to the User Registration page. You can find this option in the main navigation bar at the top of the portal. A red arrow in the provided image points to its location.

2. Fill in User Information: Complete the form by entering the new user's details into the designated fields. Fields marked with a red asterisk (*) are mandatory.

3. Provide Authorization: Check the box next to "I am authorised by my organisation to submit this registration". This confirms that you have the authority to nominate this person for access. You cannot submit the form without checking this box.

4. Submit the Request: Once all the required information is filled in and you've checked the authorization box, click the Submit button at the bottom of the form.

After you submit the form, the registration request will be processed. The new user will be notified once their access is granted.
